早期无创正压通气治疗连枷胸合并肺挫伤

目的：探讨无创正压通气模式（NIPPV）对连枷胸合并肺挫伤的早期治疗效果。方法：回顾性分析45例连枷胸合并肺挫伤患者使用NIPPV治疗的临床资料。结果：NIPPV治疗后患者症状、氧合指数、PaO2明显改善。其中5例呼吸窘迫进行性加重，行有创正压通气（11．1％），6例死产（133％）；治疗有效率75．6（34／45）％，结论：对连枷胸合并肺挫伤患者NIPPV治疗能起到胸廓内固定同时治疗ALI，ARDS的双重作用．必须警惕患者病情变化，及时调整治疗方案，以降低此类患者死亡率。

Teatment of NIPPV in flail chest together with pulmonary contusion

Objective:To discuss the effect of taking noninvasive positive pressure ventilation(NIPPV)on flail chest together with pulmonary contusion.Methods:A clinica1restrospective analysis was carried out to investigate 45 cases of flail chest together with pulmonary contusion be treated with NIPPV.Results:The symptom,PaO2 and PaO2/FiO2 of the patients were significantly better after NIPPV.Five patients aggravated acute respiratory distress syndrome(ARDS)were treated with mechanical ventilation (11.1%);6 patients died(13.33%).Treatment effective rate was 75.56 (34/45). Conclusion:To flail chest together with pulmonary contusion Intra-tranchea stabilization can be provided with NIP-PV,And acute lung injury and acute respiratory distress syndrome can be treated with it. We must have precautions on the patients condition of multiple injury. Adjusting the therapy in time is very important.
