腹腔镜脾切除术治疗外伤性脾破裂的疗效分析

目的 探讨腹腔镜脾切除术(laparoscopic splenectomy,LS)对于外伤性脾破裂的治疗效果.方法 回顾性分析2014年3月至2021年6月大连医科大学附属第二医院急腹症外科收治的35例外伤性脾破裂病人的临床资料,其中10例行腹腔镜脾切除术(研究组),25例行经腹脾切除术(对照组).比较两组病人手术方式、手术时间、术后排气时间、术后恢复进食时间、住院时间、每日负压引流量、围手术期并发症的差异.结果 与对照组相比,研究组术后恢复进食时间及住院时间明显减少(P＜0.05),在术后并发症发生率方面,研究组低于对照组,但差异无统计学意义(P＞0.05).结论 腹腔镜脾切除术治疗创伤性脾破裂,相较于传统开腹手术,具有住院时间短、术后恢复快、术后并发症少的优点,是一种安全可靠的手术方式.

Effect of laparoscopic splenectomy for the treatment of traumatic splenic rupture

Objective To investigate the therapeutic effect of laparoscopic splenectomy during the surgery of traumatic splenic rupture. Methods The clinical data of 35 patients with traumatic splenic rupture treated from Mar 2014 to Jun 2021 were analyzed. Among them, the research group consisted of 10 patients with the treatment of laparoscopic splenectomy while the control group had 25 patients who had open splenectomy. Comparing with the patients'' surgical methods,operation time, postoperative exhaust time,postoperative recovery time of food intake, length of stay,daily negative pressure drainage and postoperative complications. Results In the research group, postoperative recovery time of food intake and length of stay was significantly lower than it in the control group(P<0.05). And the incidence of postoperative complications was lower than it in the control group. But the difference was not statistically significant(P>0.05). Conclusion Laparoscopic splenectomy for traumatic splenic rupture, compared with traditional open surgery, has the advantages of shorter hospital stay, more rapid postoperative recovery and less postoperative complications. It is a safe and reliable surgical method.
