通脉愈心汤改善冠心病左室舒张功能临床观察

摘    要:对40例冠心病左室舒张功能障碍患者随机分为治疗组2 5例,对照组15例。分别给予通脉愈心汤和西药心痛定进行治疗观察,同时用多普勒超声心动仪观察左室舒张功能变化。结果:经过两组治疗前后左室舒张功能自身和组间比较,通脉愈心汤具有改善左室舒张功能作用,即改善舒张早期最大血流速度(E)、舒张早期充血时间(ET)、舒张晚期与舒张早期最大血流速度之比(A/E)的作用(P <0 . 0 1)。临床症状和血瘀证积分明显下降(P <0 . 0 5 )。提示:通脉愈心汤可通过改善左室舒张早期功能状态,提高舒张早期左室的充盈速度及充盈血流量,从而达到明显改善冠心病心气虚患者左室舒张功能的作用,使心肌供血得到改善。

Abstract:Objective:To observe the effect of Tongmai Yu xin decoction(TMYX) in treating (improving)coronary heart failure(CHD) with left ventricular diastolic dysfunction(LVDD).Methods:Forty patients with CHD and LVDD were divided into the treated group(n=25) and the control group(n=15)in the ratio of 5∶3,treated with TMYX and Nifedipin respectively.Before and after treatment,left ventricular diastolic function(LVDF) was measured by using Doppler echocardiography.Results:TMYX could improve LVDF,namely improve early diastolic peak velocity(E),early diastolic engorgement time(ET),atrium systolic and early diastolic peak velocity ratio (P<0.01).The clinical symptoms and blood stagnation scoring significantly decreased(P<0.05).Conclusion:TMYX could significantly improve CHD with LVDD and Heart Qi deficiency of TCM,and improve myocardial blood supplement through improving LVDF early condition and increasing filling velocity and filling amounts in left ventricular early diastole.
