桥小脑角病变患者继发性舌咽神经痛的围手术期护理

目的探讨继发于桥小脑角病变舌咽神经痛围手术期护理的方法和技巧。方法回顾性分析解放军总医院1996年5月-2005年6月收治的56例桥小脑角病变患者中出现的6例继发性舌咽神经痛的病例资料。6例患者均经乙状窦后入路完成手术。结果6例患者均为女性,临床主要表现咽部舌根部疼痛或伴有耳深部疼痛。6例均取得肿瘤完整切除、疼痛消失的满意疗效。其中3例术后出现轻度声嘶及饮水呛咳,经过治疗康复,平均随访2.3年,6例肿瘤及疼痛均无复发,3例声嘶呛咳患者经过治疗及正确的护理和指导后症状逐渐好转。结论桥小脑角肿瘤可引起舌咽神经痛,切除肿瘤后疼痛可消失,做好围手术期的护理可减轻患者痛苦,使患者积极配合手术。

Perioperative nursing of patients with glossopharyngeal neuralgia secondary to the pathological changes of cerebellopontine angle

Objective To investigate the method and skills of preioperative nursing of the patients with glossopharyngeal neuralgia (GPN) secondary to the pathological changes of cerebellopontine angle (CPA) disease. Methods In 56 patients with CPA from May 1996 to June 2005 in General Hospital of PLA, the information of 6 patients with GPN after CPA operation was studied. The operations were completed by retrosigmoid approach for all the patients. Results All the six patients were female, whose clinical symptoms mainly were pains at the pharynx and back of tongue. Satisfied effects of complete excision and disappearing pain were obtained in all patients. Wherein light hoarseness and choking sensation when drinking occurred in 3 patients. Caner and pain of the six patients did not recur in the following visit period with an average of two to three years through treatment and rehabilitation. The symptoms of the three patients with hoarseness and choking sensation when drinking were improved gradually through treatment and right nursing and instructions. Conclusions CPA cancer can arise GPN. The pains can disappear after the excision of cancer. Perioperative rehabilitation nursing care can decrease the pain of patients and make the patients actively cooperate with the operation.
