Survivin、PTEN在壶腹癌中的表达及意义

目的探讨Survivin和PTEN蛋白在壶腹癌组织中的表达及其临床意义。方法用免疫组化EnVision法对40例壶腹癌组织进行Survivin和PTEN蛋白的检测，并对8例正常壶腹组织进行对照研究。结果壶腹癌组织中33例Survivin蛋白呈阳性表达，阳性率为82．5％，对照组中无阳性表达，两组阳性率比较差异有统计学意义（P〈0．01）；Survivin表达与十二指肠浸润、胰腺浸润、淋巴结转移均有明显相关性（P〈0．05）。PTEN蛋白在壶腹癌组中20例阳性表达，阳性率为50％，而对照组8例均为阳性表达，两组阳性率比较差异有统计学意义（P〈0．01）；PTEN表达与十二指肠浸润、胰腺浸润、淋巴结转移均有明显相关性（P〈0．05）；经Spearman等级相关性分析表明，Survivin与PTEN蛋白表达呈负相关（r=-0．57，P〈0．01）。结论Survivin和PTEN的异常表达与壶腹癌的发生发展有关，可作为壶腹癌预后的参考指标。

Expression and significance of Survivin and PTEN in human ampullary carcinoma

Objective To study the expressions of survivin and PTEN in ampullary carcinoma and their clinical significance. Methods The expressions of Survivin and PTEN proteins were tested by EnVi- sion immunohistochemistry in 40 cases of ampullary carcinomas and 8 cases of normal ampulla of vater as control. Results The positive rate of Survivin in ampullary carcinomas was significantly higher than normal human controls （82. 5% vs 0% , P 〈0. 01 ）. The differences of the expression of Survivin in ampullary carcinoma of duodenal invasion, pancreatic invasion, and lymph node metastasis were significant （ P 〈 0.05 ）. Significant differences were also observed in the expression rate of PTEN between the patients with ampullary carcinoma and the normal controls （50% vs 100%, P 〈 0. 01 ）. The differences of the expression of PTEN in ampullary carcinoma of duodenal invasion, pancreatic invasion, and lymph node metastasis were significant （ P 〈 0.05 ）. Significantly negative correlation was found between the expression of Survivin and PTEN by using spearman correlation analysis （ r = - 0. 57, P 〈 0. 01 ）. Conclusions The abnormal expression of Survivin and PTEN gene may promote tumor genesis and progression of ampullary carcinomas and it may be used as the marker for prognosis.
