医药联合门诊肾病综合征患者药学服务路径建立及实践

目的通过建立医药联合门诊肾病综合征患者药学服务工作路径,保证肾病综合征患者药学服务工作的规范化。方法建立肾病综合征患者药学门诊服务模式,包括药物治疗评估、药物重整、用药教育、生活方式指导等药物治疗管理,并将咨询过程中涉及的专业知识点明确化。结果肾病综合征患者药物咨询问题集中于药物不良反应、疾病与饮食、用法用量3个方面,合计占比56.6%。通过制定肾病综合征患者药学路径,113例患者药物依从性、满意度提升[(5.25±1.30) vs.(6.29±1.21),80.5%vs.92.0%],并参与用药干预9例。结论建立医药联合门诊药学服务工作路径,可以增加患者药物依从性、满意度,实现专业知识及工作经验的传承、促进药学服务规范化。

Pharmaceutical services pathway and practice for nephrotic syndrome patients in physician-pharmacist joint outpatient

Objective To establish pharmaceutical services pathway for nephrotic syndrome patients in physi-cian-pharmacist joint outpatient in order to ensure the normalization of pharmaceutical services.Methods The working mode of pharmaceutical services for nephrotic syndrome patients was established,including drug therapy review,medi-cation reconciliation,medication education and life guidance.Then all of the specialized knowledge were listed.Results The medicine consultation of patients mainly focused on adverse drug reactions,disease and diet,and drug usage,ac-counting for 56.6%of the total.Through the development of pharmaceutical consultation pathway for 113 nephrotic syndrome patients,the drug compliance and satisfaction increased(5.25±1.30 vs.6.29±1.21,80.5%vs.92.0%),and 9 medication interventions were accepted.Conclusion Establishment of a pharmaceutical services pathway in phy-sician-pharmacist joint outpatient can promote drug compliance and satisfaction;professional knowledge and work expe-rience are passed on,and the standardization of pharmaceutical care will be promoted.
