摘    要:目的观察红花黄色素对老年急性脑梗死(ACI)患者中性粒细胞明胶酶相关脂质运载蛋白(NGAL)、白细胞介素-18(IL-18)和单核细胞趋化蛋白-1(MCP-1)的影响。方法选择老年ACI患者110例,随机分为观察组、对照组,各55例,两组均使用常规治疗,观察组在常规治疗基础上加用红花黄色素治疗2周。治疗前后抽取两组患者静脉血,采用ELISA法检测NGAL、IL-18和MCP-1,比较两组治疗前后中国脑卒中患者临床神经功能缺损程度评分标准(CSS)评分。结果对照组、观察组CSS评分在治疗后3 d、7 d、14 d下降(P<0.05或P<0.01);治疗后7 d、14 d,观察组CSS评分低于对照组(P<0.05)。治疗14 d,对照组NGAL、IL-18、MCP-1水平降低(P<0.05);治疗7 d、14 d时,观察组NGAL、IL-18、MCP-1水平降低(P<0.05或P<0.01);观察组NGAL在治疗7 d、14 d时低于对照组(P<0.05或P<0.01),IL-18、MCP-1在治疗后14 d时低于对照组(P<0.05)。观察组的临床治疗总有效率明显高于对照组(92.73%vs.78.18%,P<0.05)。结论红花黄色素可能通过降低老年ACI患者NGAL、IL-18和MCP-1水平,改善CSS评分,进而改善ACI患者神经功能缺损症状。

Abstract:Objective To observe the effects of safflower yellow on neutrophil gelatin-associated lipid delivery protein(NGAL),interleukin-18(IL-18)and monocyte chemoattractant protein-1(MCP-1)in elderly patients with acute cerebral infarction(ACI).Methods Totally 110 elderly patients with ACI were randomly divided into observation group and control group with 55 cases in each group.Both groups received conventional treatment,and observation group was treated with safflower yellow for 2 weeks.The venous blood was extracted from the two groups before and after treatment to detect NGAL,IL-18 and MCP-1 by ELISA method,and the clinical neurological impairment scoring(CSS)score of Chinese stroke patients before and after treatment was compared between the two groups.Results The CSS scores of patients were decreased at 3 d,7 d and 14 d after treatment(P<0.05 or P<0.01);compared with control group,CSS score in observation group decreased significantly at 7 d and 14 d after treatment(P<0.05).The levels of NGAL,IL-18 and MCP-1 in control group decreased significantly at 14 d after treatment,while those in observation group were decreased at 7 d and 14 d after treatment(P<0.05 or P<0.01);the level of NGAL in observation group was lower than that in control group at 7 d and 14 d after treatment(P<0.05 or P<0.01),and the levels of IL-18 and MCP-1 were lower than those in control group at 14 d after treatment(P<0.05).The total effective rate of clinical treatment of observation group was higher than that of control group(92.73%vs.78.18%,P<0.05).Conclusion Safflower yellow may play a role in improving the symptoms of neurological impairment of ACI patients by reducing NGAL,IL-18 and MCP-1 levels in elderly ACI patients and improving CSS scores.
