HPV感染及负荷量对宫颈癌前病变及宫颈癌的作用研究

摘    要:目的探讨人乳头瘤病毒感染(HPV)及负荷量对宫颈癌前病变及宫颈癌的作用。方法选择2009年3月-2012年3月在新疆医科大学附属肿瘤医院经病理检查确诊的宫颈病变患者1326例为研究对象,其中慢性宫颈炎1000例,CINI52例,CINⅡ40例,CINII142例,宫颈癌92例,采用杂交捕获二代(HC-Ⅱ)方法定量检测宫颈HPV-DNA的含量。结果(1)慢性宫颈炎组、CIN组、宫颈癌组HPV~DNA阳性率分别为13.5%、67.9%、94.5%,各组阳性率随着宫颈病变的程度加重而增加(P〈0.001)。(2)CINI组、CINⅡ组、CNIII组中HPV-DNA阳性率分别为46.2%、57.5%、78.8%,各组阳性率随着CIN级别增高而增高(P〈0.001)。(3)在HPV—DNA负荷量〉500时,HPV—DNA病毒负荷量随着宫颈病变程度的加重而增加,组间比较差异有统计学意义(P〈0.001)。结论HPV-DNA病毒感染及其负荷量与宫颈癌前病变及宫颈癌发生有关;HC-Ⅱ检测对预测CIN、宫颈癌发展有重要意义。

Abstract:Objective To explore HPV infection and loads on cervical carcinoma and Cervical Intraepithelial neoplasia. Methods 1 326 cervical cancer sufferers were confirmed by pathological examina- tion from march 2009 to march 2012 and were taken as subjects, including 1 000 chronic cervicitis cases, 52 CINI cases, 40 CIN eases, 142 CINIII eases, 92 cervical cancer cases. Results (1) The rate of HR- HPV infection was 13.5~//00 in cervicitis, 67.9% in CIN, 94.5% in Cervical Cancer, each positive rate in- creased as the degree of cervical lesion increases (P ~0.001); (2) The rate of HR-HPV infection was 46.2G in CIN I , 57.5G in CIN 11 , 78.8G in CIN HI, each positive rate increased as the CIN level increased (P ~ 0.001)~ (3) The comparison among four groups show that the distribution of HPV quantity was significantly different if HPV-DNA〉500 (P 〈0.001). Conclusion The viral loads of HPV DNA have a correlation with the grade of Cervial Cancer, HC-Ⅱ test has important significance in CIN and the cervical cancer screening.
