医保患者住院常见病的医疗费用及影响因素分析

目的: 评价城镇职工基本医疗保险制度实施情况,阐明住院费用的影响因素.方法: 资料来源于西安市两所综合医院2002-10/2003-10医保、公费和自费住院患者病案首页资料,应用SPSS12.0软件包进行非参检验、多元回归分析.结果: 参加基本医疗保险的城镇职工住院费用明显降低,药品费用得到控制是费用降低的主要原因.逐步回归结果表明: 住院天数、手术与否、年龄等因素影响住院费用.结论: 城镇职工基本医疗保险制度的实施有效降低了医保患者的住院费用,住院天数是影响住院费用的最主要因素.按基本用药目录给药、缩短平均住院日是控制住院费用增长的重要途径.

Study on insured inpatient's expense and influence factors of six diseases

AIM: To evaluate the basically medical insurance program and clarify the influence factors of the inpatient's expenses.METHODS: Two largest hospitals in Xi'an charging the medical insurance duties were chosen.Using SPSS12.0 package,the data of first pages of medical records from 2002/10 to 2003/10 were statistically described, including multiple regression analysis and nonparametric test.RESULTS: After the urban employees medicare system launched,the hospitalization cost of insured decreased significantly,as control of medicine fee was the most important reason.The factors influencing the inpatient's expenses were duration of hospitalization,operation or not,age and so on.CONCLUSION: The hospitalization cost is decreased in the insured inpatients.The duration of hospitalization is the most important influence factor of the cost.So using the medicine according to the basic medicine catalog and shortening the average inpatient days are important approaches to control the expenses.
