危重创伤合并DIC的评分筛选诊断

目的明确引进日本现行DIC临床评分诊断标准的意义及其优于国内现行标准的价值。方法采用日本现行DIC临床评分诊断标准及中华血液学学会DIC诊断标准对人住我院创伤急救中心的481例多发伤、多发骨折患者进行筛选诊断。在采用日方标准时，其中FDP定量一项以乙醇凝胶试验及3P试验间接判断。由此可能使本组病例的DIC积分比应得积分低0－3分，使日方标准的检出率降低。结果采用日方标准共确诊DIC11例，对此11例所做的13次诊断中均确诊为DIC；而采用国内标准只能确诊6次，漏诊7次，证实日方标准显著优于国内标准（P＜0．025）。结论推广使用日本DIC评分诊断标准将提高DIC的早期临床诊断率，适于对DIC的变化、治疗进行量化判断。如能完善FDP直接定量测定及有关辅助检验，将充分发挥该标难的效能，提高DIC检出率。

A Screening Study on the Severely Injured Complicated with DIC by a Diagnostic Scoring Scale

Objective To compar the efficacy of Japanese DIC diagncotic scoring scale to the diagnoctic system 0f Chinese Hematolop Academy. Methods DIC was screened both by the Japanere DIC diagnodic scoring scale and the diagnostic system of Chinese Hernatology Academy in 481 severely injured patients. Results Eleven cases were identified as DIC in a total of 13 diagnoses by the Japanese scoring scale, while Oniy 6 cases in 13 times were diagnosed by the domestic systam. The difference was significant(P< 0. 025). Conclusion The Japanese scoring scoring scale is superior to the domestic system and suitable for the orly diagnosis of DIC.
