CT定位小骨窗微创伤手术救治高血压脑出血

目的：总结了小骨穿开颅治疗高血压脑出血的经验。方法：ＣＴ定位小切口，小骨窗开颅，显微镜下清除脑内血肿。术中强调微创伤损伤，减少对脑深部重要结构的损伤。结果：３０例病人术后死亡３例，死亡率为１０．０％，１年随访，基本痊愈５例，显著进步１６例，进步６例。结论：在正确选择适应证的前提下，小骨窗微创伤手术治疗高血压脑出血，效果满意，死亡率明显下降。

THE CLINICAL ANALYSIS ON CURING HYPERTENSIVE APOPLEXY OF BRAIN WITH THE MINIMAL INJURY OPERATION BY CT FIXED POSITION

The experiences of curing hypertensive apoplexy of brain with craniotomy of the small window of skull were summarized. Method: Under microscope, the intracerebral hematoma was cleard away by craniotomy of the small window of skull and a small incision on CT-fixed position. In order to reduce damage of the importmant structures in the depth of the brain, the minimal injury operation was emphasized during the operation. Results:Only 3 cases died in 30 cases of the operated patients, the mortality rate was 10. 0%. 5 cases of patients recovered basically. 16 cases of patients improved remarkably. 6 cases of patients improved after following up 1 year. Conclution: Under the condition of selecting the adaptable symptom properly, the effect issatisfactory to treat the hypertensive apoplexy of brain with the minimal injury operation of the small window of skull, and the mortality rate is decreased obviously.
