强脉冲光对中厚皮片供区瘢痕预防性治疗的临床疗效观察

目的探讨强脉冲光（intense pulse light,IPL）对中厚皮片供区瘢痕预防性治疗的疗效。方法将38例自体大张中厚皮片移植术的烧伤或整形患者被分为两组：IPL治疗组21例,对照组例17例。皮片供区自然上皮化愈合后,治疗组采用IPL对中厚皮片供区瘢痕进行预防性治疗,每次间隔3～4周,治疗4～5次,并以弹力衣或弹力套压迫,外贴瘢痕敌胶片等行辅助性抗瘢痕治疗;对照组只辅助性进行抗瘢痕治疗,不做IPL治疗。治疗后以温哥华瘢痕量表对供皮区瘢痕进行评分,评估疗效。结果随访12个月,IPL治疗组中厚皮片供区增生瘢痕逐渐变平滑,色素沉着逐渐消退;对照组出现明显瘢痕增生和色素沉着。两组温哥华瘢痕量表评分比较差异有显著意义（P〈0.05）。结论 IPL是一种有效的防治中厚皮片供区瘢痕增生和色素沉着的治疗方法,操作简单,不良反应少。

Preventing Scarring in the Donor Site of Intermediate Split-Thickness Skin with Intense Pulse Light: Clinical Observation on Curative Effects

Objective To investigate the effect of intensive pulse light(IPL) to prevent the scarring in donor sites of intermediate split-thickness skin. Methods Thirty-eight burned patients or plastic surgery receivers with grafted intermediate split-thickness skin were divided into two groups: the IPL group and control group.The IPL group consists of 21 patients and the control group 17 patients.Upon the healing of the wound in the donor sites of the intermediate split-thickness skin,the patients in the IPL group were treated with the Queen IPL system and given the scarring-preventing therapy.The treatment was conducted 4 or 5 times in total with an interval of 3 or 4 weeks.The patients in the control group were given solely the scarring-preventing therapy. Results The follow-up in 12 months revealed that the appearance of hypertrophic scar of the donor sites in the intermediate split-thickness skin was better improved in the IPL group than in the control group,which is evidenced by flatter surface and less hyperpigmentation. Conclusions The IPL is effective,useful and simple in preventing the scarring and hyperpigmentation of donor sites in intermediate split-thickness skin and has few complications.
