经鼻气道正压通气与暴发呼吸运动治疗阻塞性睡眠呼吸暂停低通气综合征的血气与肺功能变化

目的探讨经鼻持续气道正压通气(n-CPAP)与暴发呼吸运动治疗阻塞性睡眠呼吸暂停低通气综合征(OS-AHS)的效果。方法对45例经多导睡眠图(PSG)确诊的OSAHS患者,比较采用n-CPAP与暴发呼吸运动治疗1个月前后的临床症状、PSG结果、血气分析指标及肺功能的变化。结果 45例患者经n-CPAP与暴发呼吸运动治疗后,夜间打鼾、白天嗜睡、乏力明显改善;夜间最低血氧饱和度(LSaO2)、平均血氧饱和度(MSaO2)均升高(P<0.01),睡眠呼吸暂停低通气指数(AHI)下降,SaO2、PaO2升高,PaCO2降低,对比治疗前后的肺功能差异有显著性(P<0.05)。结论 n-CPAP与暴发呼吸运动是中重度OSAHS患者的有效治疗方法,值得推广。

Nasal positive airway pressure combined with an outbreak of respiratory motion for treatment of obstructive sleep apnea hypopnea syndrome resulting in pulmonary function and blood gas amelioration

Objective To evaluate the clinical outcomes of the nasal continuous positive airway pressure(n-CPAP) combined with an outbreak of respiratory movement in treatment of obstructive sleep apnea hypopnea syndrome(OSAHS).Methods 45 patients with OSAHS proven by polysomnography(PSG) were enrolled in this study.The comparison of the clinical symptoms,blood gas analysis and pulmonary function changes and PSG presentation between pre-treatment and 1 month after treatment with n-CPAP combined with an outbreak of respiratory movement were performed.Results After receiving n-CPAP combined with an outbreak of respiratory motion treatment,all 45 patients in symptoms of night snoring,daytime sleepiness,fatigue ameliorated significantly;night lowest arterial oxygen saturation(LSaO2) and the mean arterial oxygen saturation(MSaO2) values all elevated(P<0.01);sleep apnea-hypopnea index(AHI) decreased;SaO2 and PaO2 values all elevated,PaCO2 decreased;in comparison of lung function between post-treatment with pre-treatment,there were significant differences(P<0.05).Conclusion The result of n-CPAP combined with an outbreak of respiratory movement for OSAHS is satisfactory,it is worth of recommendation.
