父亲血清中HBV-DNA载量与HBV父婴垂直传播的相关性分析

摘    要:目的探讨感染乙型肝炎病毒(HBV)的父亲血清中乙型肝炎病毒脱氧核糖核酸(HBV-DNA)载量对其新生儿是否携带HBV-DNA的影响,以明确需要阻断HBV父婴垂直的人群。方法以父亲乙肝病毒表面抗原(HBs Ag)阳性,母亲乙肝病毒表面抗原(HBs Ag)以及HBV-DNA均阴性的56对夫妻作为研究对象,收集夫妻双方血清以及新生儿的脐带血血清,检测血清中乙型肝炎标志物、HBV-DNA载量。以新生儿HBV-DNA载量结果作为分组依据,比较两组父亲HBV-DNA载量。结果以新生儿HBV-DNA载量结果作为分组依据,新生儿血清HBV-DNA阳性父亲HBV-DNA的载量显著高于新生儿HBV-DNA阴性者父亲HBV-DNA的载量(P0.01)。结论男性血清HBV-DNA阳性并且载量106copies/m L是HBV的父婴垂直传播的重要影响因素,可以通过孕前降低男性血清中HBV-DNA的病毒载量来降低HBV垂直传播的几率。

Abstract:Objective To explore the correlation between the HBV-DNA load of father and vertical transmission of hepatitis B virus (HBV) from father-to-neonate, so as to find out the population to prevent the HBV vertical transmission from father to neonate.Methods A total of 56 families of the father with positive HBsAg but the pregnant women with negative HBsAg and HBV-DNA were selected as the research objects and the serum samples of the couples and their neonatal umbilical blood were collected and the serum hepatitis B markers (HBVM) and HBV-DNA loads were detected.Based on the results of neonatal HBV-DNA load, the fathers were divided into two groups and their HBV-DNA loads were compared.Results The HBV-DNA loads of the fathers whose neonates with positive HBV-DNA were significantly higher than those of the fathers whose neonates with negative HBV-DNA, and there was a significant difference between the two groups (P<0.01).Conclusion The HBV-DNA load >106 copies/mL of the father is an important influencing factor in vertical transmission from father to neonate, and therefore, decreasing the load of HBV-DNA of the father before his wife with pregnancy can reduce the risk of vertical transmission of HBV.
