髋关节人工假体置换术后感染与对策

目的： 为了寻找髋关节人工假体置换术后感染最佳对策。方法： 对１９７６ 年１０ 月～１９９８ 年２ 月治疗的髋关节人工假体置换术后感染１０ 例进行回顾性分析。结果： １０ 例中７ 例属于早期感染， 其中１ 例死于糖尿病、２ 例经闭合冲洗感染得以控制、３ 例翻修、１ 例融合。１０ 例中３ 例晚期感染闭合冲洗无效。感染假体是否取出要看有无松动。最好对策还是加强预防。

Postoperative Infection of Hip prosthesis Replacement and Related management

To seek the best way to prevent the postoperative infection of hip prosthesis replacement and to improve the operative outcome.Methods:Retrospective analysis was made for 10 cases with postoperative infection of hip prosthesis replacement that were performed between October 1976 and February 1998.Results:The early infection occurred in 7 out of 10 patients.One patient died of diabets,the infection was cured and the prosthesis survived by closed irringation in two patients.The prosthesis removal with revision surgery was preformed in three patients and with arthrodesis in one patient.The late infection occurred in 3 patients and 2 of them underwent primary closed irrigation which failed to control the infection and then received prosthesis revision and arthrodesis respectively.Conclusion:the closed irrigation is effective for the early infection and ineffective for the late infection.Removal of infected prosthesis depends on its looseness,prevention is the best way for management of postoperative infection of hip prosthesis replacement.
