小切口超声乳化术中后囊膜破裂的处理

摘    要:目的 :为提高小切口超声乳化后囊膜破裂后植入折叠式人工晶体的成功率。方法 :本文观察和比较了 2 96眼小切口手术中 6眼后囊膜破裂后的三种不同处理方法。结果 :采用粘弹剂屏蔽加干性玻璃体切割的三例 ,全部在囊袋内植入了折叠式人工晶体 ,而非同轴玻璃体切割和玻璃体剪除加Simcoe针吸法只能扩大切口作PMMA硬性晶体睫状沟和前房型人工晶体植入。结论 :粘弹剂屏蔽加干性玻璃体切割的方法有可能在操作中不明显扩大后囊膜破口 ,保持了折叠式人工晶体囊袋内植入的机会 ,是一种比较安全和有效的方法

Abstract:To find the optimal way in dealing with posterior capsular rent happened in small incision phacoemulsification. Methods:In 296 eyes,6 eyes with posterior capsular rent during the surgery treated with three different techniques (manual aspiration,non coaxial vitrectomy and dry vitrectomy plus shielding with viscoelastic material) in order to receive further foldable lens implantation were studied and compared. Results:The size of posterior capsular rents kept stable and the foldable lens in bag insertion was still continued in all three cases in which the dry vitrectomy technique and compartment of viscoelastic material were used.The sulcus or anterior chamber insertion with PMMA lens was performed in remaining three cases due to insecure capsular support using non coaxial vitrectomy and manual aspiration. Conclusions:The technique of dry vitrectomy and shielding with viscoelastic material is an effective and safe way to deal with posterior capsular rent,if a foldable lens in bag insertion must be continued.
