HCG联合孕酮行黄体支持对低OHSS风险患者IVF-ET结局的影响

目的:研究体外受精-胚胎移植(IVF-ET)后孕酮(P)联合绒毛膜促性腺激素(HCG)支持黄体对低卵巢过度刺激综合征(OHSS)风险患者妊娠结局的影响。方法:140名接受IVF-ET助孕的低OHSS风险女性不孕患者,根据黄体支持方案随机分成P+HCG组和P组。化学发光法测定注射HCG日(d0)、ET前日(d4)、ET后第3日(d8)患者血清E2、P水平。比较两组种植率、临床妊娠率、流产率。同时比较妊娠组与非妊娠组d4、d8的E2、P水平。结果:P组和P+HCG组的临床妊娠率、胚胎种植率及流产率均无显著统计学差异(P>0.05)。P+HCG组d4、d8的E2水平显著高于P组(P<0.05)。妊娠组与非妊娠组d4、d8的E2与P水平无显著统计学差异(P>0.05)。结论:对低OHSS风险患者IVF-ET后加用HCG行黄体支持不影响IVF-ET妊娠率和种植率,但可以减少早期妊娠丢失率。

Effects of Human Chorionic Gonadotropin Combined Progesterone as Luteal Support on the Outcome of IVF-ET in Patients with Low OHSS Risk

Objective To investigate the effect of HCG combined with progesterone as luteal support on the outcome of IVF- ET in patients with low OHSS risk. Methods IVF-ET was performed in 140 female infertility patients with low OHSS risk, and then all the patients were randomly divided into group P ＋ HCG and group P based on the different luteal support methods. The serum levels of E2 and P were determined with immunoassay on the day of HCG administration （day 0）, day 4 and day 8. The implantation rate, pregnancy rate and abortion rate were compared between the two groups. The levels of E2 and P on day 4 and day 8 were compared between pregnant group and non-pregnant group. ResultsThere was no significant difference in pregnancy rate, implantation rate and abortion rate between group P and group P ＋ HCG（ P 〉 0.05 ）. The E2 level in group P ＋ HCG on day 4 and day 8 were all significantly higher than that in group P（ P 〈0.05 ）. The levels of E2 and P on day 4 and day 8 had no significant difference between pregnant group and non-pregnant group（ P 〉 0.05 ）. Conclusion HCG combined with progesterone as luteal support may have no effect on the outcome of IVF-ET in patients with low OHSS risk,but could decrease the abortion rate in early pregnancy.
