高强度聚焦超声(HIFU)对良性前列腺增生症患者的临床价值

目的:探讨高强度聚焦超声(HIFU)治疗良性前列腺增生症(BPH)的临床意义及疗效观察。方法:对303例不能耐受手术又不愿接受膀胱造瘘术的BPH患者行HIFU治疗。结果:303例BPH患者中,治愈207例,明显缓解53例,轻度改善34例,无效9例,总有效率97.03%。治疗后IPSS、Qmax、Volume、PVRV明显优于治疗前,各项差异均有统计学意义,治疗中所有患者均出现血尿,耻骨联合处隐痛72例,均未影响治疗,2周内症状消失,急性附睾炎7例,给予抗感染处理后治愈。结论:HIFU对治疗不能耐受手术的老年BPH患者是一种无创、无痛、安全有效的治疗方法,近期疗效显著,远期效果有待进一步观察。

High Intensity Focused Ultrasound (HIFU) on the Clinical Value of Patients with Benign Prostatic Hyperplasia

Objective Clinical significance and therapeutic efficacy of high intensity focused ultrasound in the treatment of benign prostatic hyperplasia. Methods In 303 cases of intolerance and unwilling to accept operation bladder fistulization in HIFU patients treated with BPH. Results In 303 BPH patients,207 cases were cured,34 cases markedly relieved in 53 cases,slightly improved,9 cases ineffective,the total efficiency of97. 03%. IPSS,Qmax,Volume,PVRV significantly effect after than before treatment,the difference was statistically significant,in the treatment of all patients were hematuria,pubic symphysis pain in 72 cases,were not affected by treatment,symptoms disappeared within 2 weeks,7 patients with epididymitis,give the anti infection treatment cured after. Conclusion HIFU for surgical treatment can not be tolerated in the elderly patients with BPH is a non-invasive,painless,safe and effective method of treatment,a significant short-term effect,the long-term effect needs further observation.
