老年缺血性卒中患者的认知功能与代谢综合征及炎症反应的关系

摘    要:目的探讨老年缺血性脑卒中患者的认知功能与代谢综合征(MS)及炎症反应的关系。方法94例缺血性脑卒中患者分为认知功能正常组(NC,34例),轻度血管性认知功能障碍组(MVCI,37例),血管性痴呆组(VD,23例);分别测定其空腹血糖(FBG)、空腹胰岛素(FI NS)、C反应蛋白(CRP)及血压、血脂、体质量指数(BMI),观察各组代谢综合征的发生率,检测炎症标记物,进行对比研究。结果NC组、MVCI组、VD组代谢综合征的发生率(12、33、49%,P<0.05)显著递增;CRP:MVCI组(1.4±0.5)mg/L与VD组(2.1±0.7)mg/L高于NC组(0.8±0.4)mg/L(P<0.05);CRP与胰岛素抵抗指数显著相关(r=0.39,P<0.05)。结论MS及炎症反应是老年缺血性脑卒中患者认知功能下降的重要危险因素。

Abstract:Objective To study the risk of cognitive decline in elderly patients with ischemic stroke and the metabolic syndrome,inflammation.Methods Ninty-four elderly patients with ischemic stroke were divided into normal cognition(NC) group(34 cases),mild vascular cognitive impairment(MVCI) group(37 cases) and vascular disease(VD) group(23 cases);Their fasting blood glucose(FBG),insulin levels before breakfast,serum C reactive protein(CRP) and blood pressure,lipids,body mass index(BMI) were assessed.The incidence of metabolic syndrome and the marker of inflammation of each group were observed and compared.Results The prevalence of metabolic syndrome(12,33,49% respectively) had a progressive increase among NC group,MVCI group and VD group(P<0.05),The similar results were observed in the detection of CRP(P<0.05).Conclusion MS and inflammation are important risk factors for the cognitive impairment in elderly patients with ischemic stroke.
