颈动脉内中膜厚度对冠心病的预测研究

目的通过二维超声分析冠心病患者颈总动脉（CCA）内中膜厚度（IMT）情况，探讨IMT对冠心病的预测价值。方法据冠状动脉造影将患者分为冠心病组和对照组，应用超声测量CCA—IMT。选用ROC曲线确定IMT预测冠心病的最适临界值。结果对照组及冠心病组IMT分别为（0．79±0．11）mm及（1．11±O．37）mm，两组间比较差异均具有显著性（P〈0．001），其中单支病变组和多支病变组IMT分别为（0．99±0．21）mm及（1．19±0．11）mm，两亚组间差异亦有统计学意义（P〈0．05）。通过ROC曲线确定的CCA—IMT临界值是0．90mm，此临界值预测冠心病的灵敏度为75％，特异度为87．5％。结论CCA—IMT增厚可以反映冠状动脉粥样硬化，IMT≥0．90mm对预测冠心病有一定临床意义，值得在基层医院推广应用。

Carotid intima-media thickness in prediction of coronary atery disease

Objective To explore the predictive value of common carotid intima-media thickness(IMT)detected by two-dimensional ultrasonography for coronary artery disease(CAD). Methods The patients were divided into CAD group and control group according to the findings of coronary angiography(CAG). IMT of common carotid artery was measured by sonography.Cutoff value of IMT was determined by the ROC curve.Results There were significant differences between CAD group and control group(P＜0.001)and between one-vessel group and multi-vessel group(P＜0.05).Cutoff value of IMT was 0.90 mm.The predicting sensitivity and specificity of IMT were75%and 87.5%.Conclusions Carotid carotid intima-media thickness reflects the extent of coronary artery atherosclerosis. An IMT of=0.9 mm has clinical values in predicting coronary atherosclerosis and is worth popularizing the basic hospitals.
