外围型非小细胞肺癌影像学表现与其癌细胞ras p21表达的关系探讨

摘    要:目的 探讨非小细胞肺癌的影像学表现与其癌细胞rasp2 1表达的关系。资料与方法 利用免疫组化LSAB法测定经手术及病理证实的 46例外围型非小细胞肺癌组织中rasp2 1的表达强度 ,分析其表达特点与影像学征象的相关关系。结果 外围型非小细胞肺癌的临床分期与癌细胞rasp2 1表达强度密切相关 (P <0 .0 1 ) ;肿块大小在rasp2 1表达阳性组和阴性组的出现率具有显著性差异 (P <0 .0 1 ) ,肿块毛刺征、三级支气管受累在rasp2 1表达阳性组的出现率明显高于阴性组 (P <0 .0 1 )。但分叶征、胸膜凹陷征及肺门纵隔淋巴结肿大在rasp2 1表达阳性组、阴性组的出现率无显著性差异 (P >0 .0 5)。结论 外围型非小细胞肺癌的某些影像学表现如肿块毛刺征、三级支气管受累及肿块大小等与其癌细胞rasp2 1的表达具有密切的关系

Abstract:Objective To explore the correlation of imaging characteristics of non small cell lung carcinoma (NSCLC) with ras p21 expression in lung cancer cells.Materials and Methods Immunohistochemical technique (LSAB method) was used to evaluate the expression level of ras p21 in lung cancer cells in 46 cases with pathologically proved peripheral NSCLC. The expression of ras p21 was correlated with the imaging features.Results The clinical staging of peripheral NSCLC was closely related to the expression of ras p21 in lung cancer cells. Significant difference in tumor size existed between ras p21 positive group and ras p21 negative group (P<0.01). The incidence of spicular sign, involvement of tertiary bronchus was significantly higher in ras p21 positive group than in ras p21 negative group (P<0.01). No significant difference in the incidence of lobulated sign, pleural indentation sign and hilar or mediastinal lymphadenopathy was found between ras p21 positive group and ras p21 negative group (P>0.05). Conclusion In peripheral NSCLC, certain imaging signs such as spicular sign, invasion of tertiary bronchus and the tumor size, etc. were closely related to the expression of ras p21.
