青霉素过敏反应与HLA-DRB基因多态性

摘    要:目的探讨青霉素发生过敏反应与HLA-DRB基因多态性的相关性。方法以河南汉族临床青霉素过敏反应的病人为研究对象,采用序列特异性引物-聚合酶链反应(PCR-SSP)的方法测定HLA-DRB等位基因。结果过敏病人中的DR9基因频率高于对照组(11.04%vs4.02%,RR=3.24,P<0.05),并且在速发型过敏组和荨麻疹组中均明显升高(12.16%vs4.02%,13.51%vs4.02%,P<0.05)。而DR17基因频率在迟发型过敏组和荨麻疹组中升高(16.25%vs8.05%,18.92%vs8.05%,P<0.05)。结论DR9基因可能是青霉素药物过敏反应的易感基因,并且与发生速发型过敏反应和出现荨麻疹症状有关。

Abstract:Aim To explore the correlationship of allergy to penicillin and HLA-DRB genetic polymorphism.Methods Sequence specific primer-polymerase chain reaction(PCR-SSP)was used to type for human leucocyte antigen-DBR(HLA-DRB)alleles in 77 patients with allergy to penicillin and 87 healthy subjects without any allergic reactions.Results Compared with control subjects,a significantly increased frequency of DR9 was present in 77 patients with allergic reactions(11.04% vs 4.02%,RR=3.24,P<0.05),with immediate reactions and with urticaria(12.16% vs 4.02%,13.51% vs 4.02%,P<0.05).Similarly,a significantly increased frequency of DR17 was found in patients with nonimmediate reactions and with urticaria(16.25% vs 8.05%,18.92% vs 8.05%,P<0.05).Conclusion DR9 gene was associated with immediate reactions and urticaria symptoms and accordingly,it may be a susceptible gene for allergy to penicillin.
