摘    要:目的观察2型糖尿病患者血浆同型半胱氨酸(Hcy)和C反应蛋白(CRP)与颈动脉内膜中层厚度(IMT)的关系,探讨其对糖尿病患者并发动脉粥样硬化(AS)的发病机制及临床意义。方法对62例初诊2型糖尿病患者根据颈动脉IMT分为IMT正常组及IMT增厚组,分别进行血脂、血浆Hcy、CRP的测定。并选择30例健康对照组,进行比较及相关性分析。结果糖尿病组Hcy、CRP高于健康对照组(P〈0.05)。糖尿病IMT增厚组Hcy、CRP显著高于IMT正常组(P〈0.05),且糖尿病患者血Hcy和CRP与颈动脉IMT均呈正相关(P〈0.05、P〈0.01),Hcy与CRP也相关(P〈0.05)。结论血Hcy和CRP在预测2型糖尿病患者早期动脉粥样硬化有重要的临床意义,炎症反应可能是高Hcy促AS重要的机制之一。

Abstract:Objective To explore the relationship between the plasm level of homocysteine(Hcy) and C- reaction protein(CRP) level and carotic intimal- medial thickness(IMT) in type 2 diabetes(T2DM). Methods 62 patients with preliminary diagnosis of T2DM were divided into two groups according to IMT level :normal IMT group and IMT increased group . 30 healthy people were chosen as control. All patients and control were measured with Hey, CRP and total cholesterol(TC). Results Hey and CRP levels in T2DM were higher than those in controls(P 〈 0.05).Hcy and CRP levels in group of increased IMT were higher than those in normal IMT group ( P 〈 0.05). The positive correlation between Hcy, CRP levels and IMT was shown ( P 〈 0.01 ), so was the correlation between Hcy levels and CRP levels ( P 〈 0.05 ). Conclusions Hcy and CRP levels may predict the early stage of AS in T2DM clinically. Chronic inflammation may be the important molecular meehanisms of high Hcy levels causing effect on ASs formation.
