ERCP引发急性胰腺炎的高危因素探讨及对策

目的探讨ERCP术后胰腺炎的高危因素及预防对策。方法对1998年10月至2006年7月行ERCP检查及治疗的597例患者行回顾性分析。结果ERCP术后发生急性胰腺炎56例，与之相关的主要因素有年龄、多次胰管显影、插管困难重复多次和乳头括约肌切开（ERCP）及预切开等。结论ERCP术后发生急性胰腺炎与患者自身因素及操作技术有关，提高操作技术、术后鼻胆管引流（ENBO）及药物预防能有效控制ERCP术后胰腺炎的发生。

Study on the high risk factor of acute pancreatitis induced by ERCP and its prevention

Objective To explore the high risk factors of pamcreatitis after ERCP operation and its prevention , Methods A retrospective analysis was done to the 597 patients who underwent ERCP from Oct , 1998 to Jul , 2006 . Remits 56 cases had acute pancreatitis after ERCP , the main factors were age,many times of pancreatic ductography,difficulty and repeating in catheter insertion and sphincterotomy（FAT） . Conclusions The onset of acute pancreatitis after ERCP was related to both the patients＇ personal character and the procedure. Improvement of technique , postoperative nasobiliary drainage（ENBO） and preventive use of drug can effectively prevent its happening .
