血清肿瘤标记物联合检测在非小细胞肺癌诊断中的应用价值

目的探讨联合检测血清肿瘤标记物CEA、CA125、VEGF、SGF、TSGF在非小细胞肺癌(NSCLC)临床诊断中的应用价值。方法应用放射免疫法、酶联免疫法、胶乳凝集法和比色法检测98例非小细胞肺癌病人、48例肺良性病变病人(对照组1)及42例正常健康人(对照组2)的血清中CEA、CA125、VEGF、SGF和TSGF的水平。结果非小细胞肺癌组血清5项肿瘤标志物含量均高于肺良性病变组及正常组;对照组1与对照组2比较差异无显著性意义(P>0.05);随着临床分期逐步升高,CEA、CA125和VEGF阳性率水平升高(P<0.05);NSCLC患者血清CEA、CA125、VEGF、SGF和TSGF含量在治疗前后相比,有显著差异(P<0.05)。5项联合检测非小细胞肺癌的敏感性和特异性可分别达95.9%、62.5%。结论运用联合检测多种肿瘤标记物可以提高非小细胞肺癌诊断的敏感性,同时对于确定其临床分期和观察疗效具有一定的意义。

Evaluation of Clinical Value of Tumor Markers Combination Measurement in Diagnosis of Non - small Cell Lung Cancer

Objective To study the clinical value of combined measurement of CEA,CA125,VEGF,SGF and TSGF in the diagnosis of non-small cell lung cancer.Methods The serum levels of CEA,CA125,VEGF,SGF and TSGF in 98 non-small cell lung cancer patients,48 benign pulmonary diseases and 42 healthy subjects were measured by radiommunoassay,enzyme linked immunosorbent essay(ELISA),emulsion method and chemical metric analysis.Results The levels of CEA,CA125,VEGF,SGF and TSGF in non-small cell lung cancer patients were significantly higher than that in benign pulmonary diseases and healthy groups.There was no significant difference in serum level of the tumor marker panel between the benign pulmonary diseases and healthy groups(P>0.05).Positive correlation was found among the circulating levels of CEA,CA125 and VEGF expression and clinical stage(P<0.05).The levels of CEA,CA125,VEGF,SGF and TSGF in patients with non-small cell lung cancer were siginificantly different after treatment(P<0.05).Sensitivity and specificity in the combination measurement of CEA,CA125,VEGF,SGF and TSGF for non-small cell lung cancer were 95.9% and 62.5% respectively.Conclution Combined measure of multiple serum tumor markers can significantly increase the diagnosis sensitivity for non-small cell lung cancer.Meanwhile,it is also significant for defining clinical staging and improving the therapeutic effect.
