摘    要:目的探讨成人股骨头无菌性坏死(ANFH)的影像学特征,提高对本病的早期诊断水平。方法回顾性分析经临床和病理证实的40例成人ANFH的临床资料及DR、CT及MRI影像表现。结果本组40例.单侧病变30例,双侧病变10例,共有50个股骨头出现无菌性坏死。50个股骨头行DR检查,30个(60%)为阳性,20个为正常;32例40个股骨头行CT检查,28个(70%)为阳性,12个为阴性;28例31个股骨头行MRI检查,29个(94%)为阳性,2个为阴性,MRI诊断ANFHI期准确率为90%(18/20)。结论DR是ANFH的常规检查方法。CT检查是DR的重要补充;MRI的敏感性、特异性高,是诊断早期ANFH的首选方法。

Abstract:Objective To study the imaging characters of aseptic necrosis of femoral head (ANFH)in adult,to improve the early diagnostic level of ANFH. Methods The clinical data and imaging manifestation of DR,CT and MRI in 40 adults with ANFH confirmed by clinic and pathology. Results Among 40 cases,30 cases were unilateral lesions,10 cases were bilateral lesions,there were 50 femoral heads having aseptic necrosis.50 femoral heads were given DR examina- tion,among them,30 ones (60%) were positive,20 ones were normal;40 femoral heads in 32 cases were given CT exami- nation,28 ones (70%) were positive,12 ones were negative;31 femoral heads in 28 cases were given MRI examination, 29 ones (94%) were positive,2 ones were negative,and the accuracy rate of diagnosed by MRI was 90%(18/20). Conclusion DR is the general examination method for ANFH,CT examination is the important supplement for DR;MRI has high sensitiveness and specificity, and it is the preferred method for early diagnosis of ANFH.
