GHbA1c和hsCRP与高血压患者颈动脉粥样硬化程度的关系

摘    要:目的研究糖化血红蛋白A1c(glycosylated hemoglobin A1c,GHbA1c)和高敏C反应蛋白(high sensitivity reactive protein C,hsCRP)与原发性高血压患者颈动脉粥样硬化程度的关系。方法对152例原发性高血压3级患者进行颈动脉超声检查,并检测GHbA 1c和hsCRP。按不同颈动脉超声结果分组后比较各组的GHbA1c和hsCRP乘积。结果颈动脉超声检查结果合并颈动脉内膜增厚,尤其合并颈动脉斑块形成的原发性高血压患者血清hsCRP和HbA1c的乘积显著高于无颈动脉内膜增厚患者(F=8.893,P=0.001):血清hsCRP和HbA1c的乘积与原发性高血压患者颈动脉粥样硬化(IMT增厚或斑块形成)呈正相关(r=0.459。P=0.001)。结论糖耐量异常和炎症反应的联合作用,加重了原发性高血压患者动脉粥样硬化的程度。

Abstract:Objectives To investigate the joint effects of glycosylated hemoglobin A1c (GHbA1c) and highsensitivity C reactive protein (hsCRP) on carotid atherosclerosis in hypertensive subjects. Methods A total of 152 inpatients (mean age 65 years) diagnosed grade 3 hypertension were enrolled. They underwent uhrasonographic evaluation of the carotid arteries. Measurements of hsCRP and HhA1c were performed in addition to common risk factors. According to the difference results of uhrasonographic evaluation of the carotid arteries, the patients were divided into 3 groups and the cross products of hsCRP and HbA1c of them were analyzed. Results Analysis of variance revealed that the cross products of hsCRP and HbA1c in hypertensive subjects with carotid atherosclerosis or carotid artery plaque were higher than the hypertensive subjects without carotid atherosclerosis (F=8.893, P=0.001 ). In the patients diagnosed grade 3 hypertension, the cross product of hsCRP and HbA1c was positively correlated with an enhanced carotid atherosclerosis burden (r= 0.459, P = 0.001 ). Conclusions The combination of glycometabolism dysfunction and inflammation is associated with an enhanced atherosclerotic burden in hypertensive subjects.
