利培酮联合丙戊酸镁治疗脑外伤所致精神病性障碍疗效分析

目的研究利培酮联合丙戊酸镁治疗脑外伤所致精神病性障碍的治疗效果及药物不良反应。方法收集78例入住我院的脑外伤所致精神病性障碍患者，随机分为2组进行为期8周的治疗干预，一组采用利培酮联合丙戊酸镁治疗作为治疗组，一组单独采用利培酮治疗作为对照组，分别在治疗开始时、治疗1周、治疗4周、治疗8周时利用简明阳性症状与阴性症状量表（positiveandnegativesynptomseal，PANSS）、副反应量表（treatmentemergentsymptomscale，TESS）进行疗效评估，记录不良反应，并进行组问比较分析。对各指标均数比较采用组间t检验，计数资料采用组间x2检验。结果脑外伤所致精神病性障碍应用治疗显著有效率74．4％；对照组显著有效率57．2％。两组有效率比较，差异有统计学意义（P〈0．05）。观察组不良反应发生率为25．6％（10／39），对照组33．3％（13／39），两组不良反应差异无统计学意义（P〉0．05）。观察组辅助检查有肝功能异常3例、窦性心动过速5例，对照组有肝功能异常4例、窦性心动过速6例，两组辅助检查异常率差异无统计学意义（X2=0．394，P〉0．05）。结论利培酮联合丙戊酸镁治疗脑外伤所致精神病性障碍，较单一使用利培酮治疗疗效更佳，减轻对社会的危害，且不增加不良反应，值得在临床工作中推广使用。

Clinical analysis on the efficacy of risperidone combined with magnesium valproate treatment on mental disorders in patients with brain injury

Objective To investigate the efficacy and adverse reactions of risperidone combined with magnesium valproate on mental disorders in patients with brain injury.Methods From Januray 2009 to August 2011,78 cases of mental disorders caused by brain injury were recruited and divided randomly into 2 groups.One group accepted risperidone only as control.The other one accepted additional magnesium valproate treatment.The efficacy and adverse reactions of risperidone combined with magnesium valproate treatment on mental disorders caused by brain injury were analyzed in SPSS 17.0 for Windows by means comparison or Chis-square test respectively.Results The significant efficacy rate of risperidone combined with magnesium valproate treatment group vs.control group shows significant difference(74.4% vs 57.2%,P < 0.05),while the side effects rate copariosn without significant differrence(P > 0.05).Conclusion Risperidone combined with magnesium valproate on mental disorders in patients with brain injury could obtain better efficacy,reduce the possible harm to the society without increasing the adverse reaction rate,which should be widely used.
