经阴道超声检查盆腔包块漏诊分析

目的分析经阴道超声检查对盆腔包块的漏诊情况，探讨经阴道超声存在的局限性，提高超声检查厦临床的诊断符合率。方法回顾性总结49例经阴道超声检壹盆腔包块的漏诊病倒，全部经手术病理征实。分析该组病例漏诊的原因。结果经阴道超声对盆腔器官的声像图显示较经腹部超声显示更清晰，对带蒂的浆膜下子宫肌瘤、畸胎瘤、输卵管积液、位置较高的盆腔包块厦宫颈外口病灶等可能出现漏诊。结论经阴道超声检查诊断盆腔包块有较高的准确性，但经阴道超声存在一定的局限性。不能完全替代经腹部超声及妇科检查，检查时不要仅满足于子宫卵巢的正常图像，进行多切面的仔细扫查厦经腹部超声与经阴道超声二者配合能减少漏诊的发生。

Analysis of missed diagnosis in pelvic mass by transvaginal ultrasound examination

Objective To increase diagnostic accordance rate of sonography and clinical diagnosis and to analyze the missed diag- nosis percentage and the shortcoming of transvaginal ultrasound in pelvic mass.Methods Forty-nine missed diagnosis cases of pel- vic mass by transvaginal ultrasound were retrospectively summarized.All cases were confirmed by pathological diagnosis after sur- gery and causes were analyzed.Results The sonographic presentation of pelvic organ was clearer than transabdomen ultrasound. The missed diagnosis often occurred in pedicled subserosa hysteromyoma,teratoma,sactosalpinx,pelvic mass in high position and external os lesion and so on.Conclusion To pelvic mass,transvaginal ultrasound has higher accuracy;due to some limitation it can not completely take place of transabdomen ultrasound and gynecologic examination.In order to decrease missed diagnosis,it de- mands not only normogram of uterus and ovarian,but also multisectioned scan and combination of transabdomen ultrasound and transvaginal ultrasound.
