肺炎衣原体感染与脑卒中的相关性研究

摘    要:目的探讨肺炎衣原体 (CPn)感染与脑卒中的关系。方法用酶联免疫吸附试验 (ELISA)法检测急性脑卒中患者1 1 2例 (其中脑梗死患者 69例 ,脑出血患者 43例 )、其他疾病患者 50例的血清肺炎衣原体特异性IgG抗体及IgM抗体 ,以及血清总胆固醇 (TC)、甘油三酯 (TG)、高密度脂蛋白胆固醇 (HDL C)水平。结果脑梗死组、脑出血组和对照组肺炎衣原体感染率分别为 67.4%、52 .5 %、34 .0 % ,其中脑梗死组及脑出血组肺炎衣原体感染率均高于对照组 ,无统计学差异 (P 0 .0 5)。而脑梗死组与对照组则有显著性差异 (P <0 .0 1 )。肺炎衣原体IgG阳性病例血清TG浓度明显高于肺炎衣原体IgG阴性的病例 (P <0 .0 5) ;HDL C则明显低于阴性组。结论肺炎衣原体感染与脑血管病可能有明显的关系 ,尤其是脑梗死。其可能的机理是肺炎衣原体感染后形成免疫复合物沉积在血管壁 ,引起血管局部损伤 ,以及改变了血脂浓度而增加脑血管病发生的危险性

The correlation between chlamydia pneumonia infection and stroke

Abstract:Objective To explore the relationship between chlamydia pneumo nia(CPn) infection and stroke.Methods Serum specific IgG antibod ies,IgM antibodies to chlamydia pneumonia,serum concentrations of total choleste rol(TC),triglyceride(TG),and high density lipoprotein-cholesterol(HDL-C) in 11 2 cases of acute stroke(including 69 cases of cerebral infarction and 43 cases o f cerebral hemorrhage) and 50 cases with other diseases were detected by enzyme linked immunosorbent assay(ELISA).Results The chlamydia pneumoni a infection rates were 67.4%,52.5% and 34%,respectively,in the cerebral infarcti on group,cerebral hemorrhage group and control group,of which,the rate of cerebr al infarction group was higher than that of cerebral hemorrhage group,with no st atistical difference(P<0.05).There was significant difference be tween cerebral infarction group and control group(P<0.01).TG lev el in CPn IgG positive cases was obviously higher than that of IgG negative case s(P<0.05),while HDL-C was lower than that of negative cases. Conclusion The chlamydia pneumonia infection may be associated with cerebral vascular disease,especially cerebral infarction,with the possible mech anism that immunocomplex deposited at the vascular wall causes local vascular in jury and that CPn infection might modify the serum lipid concentration in a way that can increase the risk of cerebral vascular disease.
