SLE患者外周血液系统变化与自身抗体相关性分析

目的：探讨系统性红斑狼疮（SLE）血液系统受损发生率与SLE病情活动程度及自身抗体的关系。方法：分析191例SLE患者的临床资料及相关实验室指标，应用χ^2检验进行统计学分析。结果：191例SLE患者有82．20％（157例）出现血液系统损害，其中贫血占63．87％，白细胞减少占52．88％，血小板减少占30．37％。单系血细胞减少占33．51％，两系及两系以上受损占48．69％。血液系统损害组中，抗U1RNP抗体阳性率为40．13％，低于无血液系统受损组58．82％；抗心磷脂抗体（ACL）阳性率为24．84％，高于无血液系统受损组2．94％。单纯白细胞减少组和单纯血小板减少组ACL阳性率分别为23．81％和28．57％，高于无血液系统受损组2．94％。血液多系受损组抗ds-DNA抗体阳性率68．82％高于单系受损组48．44％。SLE活动组154例，非活动组37例，血液系统损害发生率SLE活动组与非活动组比较差异无统计学意义。而活动组抗ds-DNA抗体阳性率65．38％高于非活动组37．04％。结论：SLE血液系统受损与病情活动性无关：抗ds-DNA抗体和ACL可能参与了SLE血细胞破坏。

The relationship between impairment of hematological system and autoantibodies in systemic lupus erythematosus

Objective： To approach the relationship between impairment of hematological system and autoantibodies, disease activity in systemic lupus erythematosus （SLE）. Methods： 191 cases of SLE were investigated,the relationship between them was analysed with Chi-square test. Results： In 191 cases with SLE,82.20% （157 cases）patients appeared impairment of hematological system,anemia was 63.87% , leucopenia was 52.88%,and thrombocytopenia was 30.37%.Impairment of hematological mono-system was 33.51% ,while impairment of hematological multi-system was 48.69% .Positive ratio of anti-U1RNP antibody（40.13%） in group with impairment of hematological system was lower than that in the normal group （58.82%）,whereas positive ratio of ACL（24.84%） was higher than that in the normal group（2.94%）.Positive ratio of ACL in group leucopnia（23.81%） and group thrombocytopenia（28.57%） were higher than that in the normal group （2.94%）.Positive ratio of anti-ds-DNA antibody in group with impairment of hematological muhisystem （68.82%） was higher than that in the group with impairment of hematological mono-system （48.44%）.154 cases were in active disease,the other 37 cases were not.The ratio of impairment of hematological system between the active and static group was of no statistical significance,but positive ratio of anti-ds-DNA antibody in active group （65.38%） which had impairment of hematological system was higher than that in the static group（37.04%）. Conclusion： There is no relationship between the impairment of hematological system and disease activity;Anti-ds-DNA antibody and ACL might be related with impairment of hematological system in SLE.
