应用自控微动带锁髓内钉治疗股骨干骨折

摘    要:目的观察自控微动带锁髓内钉(A M L N)治疗股骨干骨折的临床效果。方法采用A M LN钉治疗股骨干骨折29例,其中横形13例,短斜形4例,螺旋形1例,粉碎性11例。结果所有患者随访8~22个月,骨折全部愈合,按K lem m功能恢复分级标准优26例,良3例。结论A M LN钉具备了坚强与弹性两类内固定器械的优点,解决了临床上坚强内固定骨折端应力传导不够或没有应力传导,弹性固定不易控制骨折端弯剪扭旋应力这一矛盾。所设计的自控微动装置使骨折局部始终处于有生理性应力传导与刺激的生物力学环境中,骨痂边生长边塑形,避免了骨折不愈合及骨折病的发生。A M LN钉及配套器械设计合理,手术操作简便,易于推广。

Abstract:Objective To observe the clinical effects of autocontrol micromove ment locking nail (AMLN) in treatment of fracture of femoral shaft. Methods 29 c ases with fracture of femoral shaft were treated with AMLN. 13 cases of them wer e transverse fracture, 4 cases oblique fracture, 1 case spiral fracture and 1 ca se comminuted fracture. Results Follow-ups for 8 to 22 months revealed that all the fractures healed. According to Klemm evaluation system, 26 cases were excel lent and 3 fine. Conclusions The reasonable design of AMLN and its auxiliary ins truments makes it easy to be used and extended. Its rigidity and elasticity can solve the problem that rigid internal fixation does not provide enough stress co nduction or provides no stress conduction in the fracture region while elastic i nternal fixation cannot control bending, shearing, torsion and rotation in the f racture region. AMLN can provide the fracture region with physical conduction of stress and stimulating biomechanical environment and make callus mould as it gr ows, which can avoid nonunion and fracture deformity.
