血栓素A2和前列环素在实验性蛛网膜下腔出血后脑血管痉挛中的作用

摘    要:目的:探讨血栓素A2(TXA2)和前列环素(PGI2)在兔蛛网膜下腔出血(SAH)致脑血管痉挛(CVS)中的作用。方法:行兔枕大池单次注血制备SAH模型,分别于注血前、注血后第1天、第4天、第7天行CTA检查,根据第4天和第7天的检查结果分为痉挛组和未痉挛组。采兔耳缘动脉血,放射免疫法测定血栓素B2(TXB2)和前列腺素F1α(PGF1α)的表达,计算TXB2/PGF1α比值。结果:未痉挛组在注血后第1天、第4天以及第7天外周血TXB2、PGF1α和TXB2/PGF1α改变不明显,与注血前比较差异均无显著性。痉挛组外周血TXB2、PGF1α第1天升高不明显,第4天和第7天均显著升高,且TXB2较PGF1α升高幅度大,TXB2、PGF1α以及TXB2/PGF1α和注血前及未痉挛组比较差异有显著性(P〈0.01)。结论:TXA2和PGI2可能没有参与急性脑血管痉挛的发生,但在迟发性脑血管痉挛发生中可能起重要作用。

Abstract:Objective: To explore the effect of TXA2 and PGI2 in acute and delayed cerebral vasospasm (CVS) after subarachnoid hemorrhage(SAH). Methods:Thirty experimental SAH models in rabbits were established by single injection of autologuous arterial blood to cisterna magna. According to the result of CTA, the rabbits were divide into spasm group and unspasm group. The concentrations of TXB2 and PGF1α in peripheral blood were examinated with radioimmunologic techniques at 1 day,4 and 7 days after SAH. Results:The concentration of TXB2 and PGF1α in spasm group showed no increasing on the 1 day after SAH, but significantly increased on 4 and 7 days(P〈0.01). On 4 and 7 days after SAH the concentration of TXB2 and PGF1α in spasm group had significant difference with unspasm group(P〈0.01). Conclusion: TXA2 and PGI2 may have no effect on the generation of acute CVS,but they may paly a role in the occurance and development of the delayed CVS process.
