硬膜外腔置管前注药预防血管损伤的临床观察

为观察连续硬膜外麻醉置管前注射法对血管损伤的预防作用，将280例行剖宫产患者随机分成两组，观察组138例置管前推注硬膜外麻醉下局麻药试验量，对照组142例置管前不推注局麻药。结果显示：观察组血管损伤6例（4．35％），对照组血管损伤18例（12．68％），两组血管损伤率差异具有显著性（P＜0．05）。提示：临床硬膜外穿刺，尤其是腹内压增高的患者，在置管前推注含肾上腺素的局麻药试验量，有利于预防血管损伤，减少硬膜外麻醉的并发症。

Clnical Observation on Epidural Injection Before Catheterization for Prevention of Epidura-Vascular Injury

Obervation of epidural injection before catheterization for prevention of epidura-vascular injury. 280 cases of cesarean section were randamly divided into two groups . Observation group : n=138. Epidural injection of lidocain in test dose was per formed before catheter insertion . Control group : 142 cases local anesthesic injection af ter epidural catheter insestion . The results showed that 6 cases of epidural vascular in jury were found in oborvation group, 18 cases were found in control group, the injury rate of control group (12. 68% ) was obviously higher than that of observaion group (4. 35%) (P<0.05), It is suggested that epidural vascular injury may be prevented by test-dose anesthetic injection (with 1:200000 adrenaline) before epidural catheten inser tion, especially it is in favour of the patients with high intra-abdominal pressure and de crease the complication from epidural anesthesia .
