摘    要:目的探讨急性心肌梗死(AMI)患者早期(〈2周)心率变异性(HRV)与恶性室性心律失常(LownⅢ级以上)的关系。方法选取60例AMI患者,按修正Lown分级标准分类,分成2组:AMI组(23例,LownⅡ级以下)和恶性组(37例,LownⅢ级以上合并AMI),发病后第2周作Holter检查和HRV分析;另选40例正常人作为对照组。结果AMI组HRV多指标明显低于对照组,AMI合并恶性心律失常组HRV明显低于AMI组。结论HRV的下降反映了AMI患者自主神经功能的损害,与恶性心律失常密切相关。

Abstract:Objective To investigate the relationship between heart rate variability (HRV) in the early period ( 〈 2 weeks) and malignant ventricular arrhythmia ( 〉 Lown grade Ⅲ) in acute myocardial infarction patients. Methods 60 patients with acute myocardial infarction were enrolled and stratified to two groups according to the modified Lown classification: 23 patients in AMI group and 37 patients in AMI with malignant ventricular arrhythmia group. 2 weeks after AMI, 24 h Hoher recording and heart - rate variability analysis were performed. 40 healthy male individuals served as the control group. Results The HRV indexes were markedly lower in AMI group than in control group, and those in AMI with malignant ventricular arrhythmia group were more depressed as compared to AMI group. Conclusion Depressed HRV could reflect the autonomic imbalance in AMI patients and was closely correlated with malignant ventricular arrhythmia.
