探讨COX-2在结肠癌中表达的临床意义及预后

目的探讨COX-2在结肠癌组织中的表达与临床病理特征及预后的关系。方法运用免疫组化法检测522例手术切除的结肠癌组织中COX-2的表达,并分析其与临床病理和预后的关系。结果 COX-2在大肠癌中阳性表达率为81.99%。在肿瘤体积≥5cm时,COX-2的阳性表达率为88.80%;在侵及质膜结肠癌组中,COX-2阳性表达率为84.67%。在伴有淋巴结转移的结肠癌中,COX-2阳性表达率为89.08%,转移组与非转移组相比较,差异有统计学意义（P〈0.05）;在C＋D期结肠癌组织中,COX-的阳性表达率为85.71%（P〈0.05）。COX-2表达与肿瘤大小,浸润深度、Duke＇s分期、是否有区域淋巴结转移有关（P〈0.05）;COX-2阳性组生存期明显低于COX-2阴性组。结论 COX-2蛋白的表达是反映结肠癌侵袭、转移的重要标记物,COX-2的检测有助于评估结肠癌的预后。

Expression of COX-2 in colorectal carcinoma and the relationship to prognosis

Objective To study the expression of cyclooxygenase-2(COX-2) in colorectal carcinoma and their correlations to the clinicopathologic factors and the relationship to prognosis.Methods The expressions of COX-2 was detected by immunohistochemical staining(S-P method) in 522 intestinal biopsy specinzens taken from patients with colorectal carcinoma.Results The positivity rates of COX-2 in colorectal carcinoma was 81.99%.When Tumor size ≥5cm,the positivity rates of COX-2 was 88.80%.The positivity rates of COX-2 were 84.67%,89.08%and 85.71%,respectively a mong patients who had the colorectal carcinoma to serous membrane he,lymph node metastasis and in stages of C and D among Dukes..There were significantly difference between the patients with lymph node metastasis and those without node involvement(P<0.05).COX-2 expression was also related to tumor size,depth of invasion,Duke's stage,and lymph node status(P<0.005).Conclusion The expressions of COX-2 may be important biological makers for reflecting the invasion and metastasis of human colorectal carcinoma.COX-2 can provide information for evaluateing colorectal carcinoma prognosis.
