乙型肝炎病毒相关性肾炎患者肾组织中HBsAg、HBcAg的检测

目的观察乙型肝炎病毒相关性肾炎(HBV-GN)患者肾脏组织中HBsAg、HBcAg的表达情况.方法以63例HBV-GN患者为研究对象,另随机选择20例非乙型肝炎病毒相关性肾炎(NHBV-GN)患者、12例血清HBV标志阳性的其他肾病(肾结核、肾结石、肾细胞癌、肾萎缩等)患者作为对照组,用免疫组化方法检测肾穿刺活检组织HBsAg、HBcAg的表达.结果 HBV-GN患者肾脏组织中均可检出HBsAg、HBcAg阳性颗粒,阳性率分别为76.2%(48/63),42.9%(27/63);20例NHBV-GN患者肾组织HBsAg、HBcAg均未检出,与HBV-GN患者相比有统计学差异(P＜0.05);12例血清学HBV标志阳性的其它肾病患者肾组织HBsAg、HBcAg阳性率均为75%,与HBV-GN患者相比无统计学差异(P＞0.05).血清HBeAg 阳性与血清HBeAg阴性的HBV-GN患者相比,肾脏组织中HBsAg、HBcAg表达情况经统计学比较,无显著性差异(P＞0.05).结论 HBV在肾脏中定位及复制,不仅可引起肾脏损害,并与病毒的保存及乙型肝炎的传播可能有关.

Detection of HBsAg and HBcAg in renal tissues from patients with HBV related glomerulonephritis

Objective: To investigate the presence of HBsAg and HBcAg in renal tissues from patients with hepatitis B virus related glomerulonephritis(HBV-GN). Methods: Renal tissue ordinary biopsies from 63 patients with HBV-GN and two control groups were prepared for immunocytochemical detection of HBsAg and HBcAg.Results:HBsAg and HBcAg were present with the positive rates of 76.2%, 42.9% respectively in renal tissues from HBV-GN patients, which were higher than those of NHBV-GN patients, but assumed no statistical difference compared with those of other renal patients with positive HBVM. Renal tissue HBsAg and HBcAg positive expression rates showed no statistically significant difference between HBV-GN patients with positive serum HBeAg and HBV-GN patients with negative serum HBeAg. Conclusions: The location and replication of HBV in renal tissue may lead to renal lesions, moreover, it plays a role in the preserve of virus and the transmission of hepatitis B.
