核磁共振胰胆管造影与内镜下逆行胰胆管造影在胰胆疾病诊断方面的对比研究

目的：探索核磁共振胰胆管造影（ＭＲＣＰ）检查在临床应用中的价值。方法：对１１０例胰胆管疾病患者选择性地行ＭＲＣＰ检查，并与内镜下逆行胰胆管造影（ＥＲＣＰ）检查作比较，结合内镜下治疗以及外科手术，以明确两者之间的相关性。ＥＲＣＰ和手术结果作为金标准。结果：１１０例患者ＭＲＣＰ图像质量均较高，对胆管扩张诊断的敏感性为８５．０６％（７８／８７），对胆管下段狭窄伴扩张诊断的敏感性为９０％（１８／２０）。Ｍ

Study of Magnetic Resonance Cholangiopancreatography Comparing with Endoscopic Retrograde Cholangiopancreatography in the Diagnosis of Bile Duct and Pancreatic Duct Diseases

Purpose: To evaluate magnetic choiangiopancreatography in the diagnosis of bile duct and pancreatic tract diseases. The purpose of this study was to determine in a prospective sensitivity and specificity of MRCP for the evaluation of bile duct and pancreatic tract abnormalities. Methods: 110 patients refered for elective MRCP comparing with ERCP or operation during nine months periods. The presence of dilatation strictures and intraductal abnormalities was recorded sensitivity and specificity of MRCP were determined using findings on ERCP or operation as the gold standard. Results: MRCP images of diagnostic quality were obtained in 110(100%) of the patients. Sensitivity for the detection of bile duct dilatation (n=87), pancreatic duct dilatation (n=30) and intraductal was 85.06%, 53.33% respectivily, sensitivity for the detection of normal bile duct and pancreatic duct was 100%. Conclusions: MRCP has a noninvasive diagnostic modality capable of producing high quality images of the bile duct and pancreatic duct. MRCP has a high sensitivity and specificity in the evaluation of the bile duct and pancreatic ducts base on the data.We believe that the efficacy of MRCP is sufficient to warrant its use in the diagnosis of bile duct and pancreatic tract diseases, but not replace ERCP.
