辨证分期中药内服熏洗促进手术整复内固定第5掌骨颈骨折愈合临床观察

[目的]观察中药辨证分期内服熏洗促进手术整复内固定第5掌骨颈骨折愈合疗效。[方法]对24例第5掌骨颈骨折进行手术复位后,依据骨科病程辨证分期促进骨折愈合。初期(骨折后1～2周炎症期和修复期第一阶段)行气消瘀桃红四物汤加减;中期(骨折后3～4周内,修复期中期)和营止痛、接骨续筋,续骨活血汤加减;后期(骨折后1个月修复后期)补气养血、补益肝肾,八珍汤及补肾壮骨汤加减(党参、白术、茯苓各10g,炙甘草5g,川芎6g,当归、熟地、白芍、牛膝各10g,山萸、杜仲、续断各12g,生姜3片,大枣2枚)1剂/d,水煎1000mL,3次/d口服。恢复期(外固定物解除后功能锻炼阶段)中药熏洗(艾叶散100g+1000mL,温洗患处)每次30min,3次/d,10d为1个疗程。观测临床症状、第5掌指关节活动范围。随访6个月观测第5掌指关节活动范围。24例随访6个月,第5掌指关节活动范围78.2°。[结果]24例患者术后拍X线片示,达到解剖复位18例,近解剖复位6例。术后4～6周达到临床愈合,平均4.7周,骨折临床愈合后拔出克氏针,8例患者3周去除石膏托。[结论]中药辨证分期内服熏洗可促进手术整复内固定第5掌骨颈骨折愈合,值得推广。

Manipulation and Fixation Method with Herbal Medicine Treating the Fifth Metacarpal Neck Fracture of Clinical Observation

[Objective] To introduce manipulation and fixation method with herbal medicine treating the fifth metacarpal neck fracture.[Methods]After make 24 cases neck fracture of metacarpal bone reached anatomical location,Kirschner wire internal fixation of 4~6 week combined with traditional Chinese medicine treatment.[Results]24 patients,4~6 weeks to achieve clinical union after surgery,Postoperative follow-up 6~12 months,the movement of metacarpophalangeal joint reach to 78.5.[Conclusion]This kind of fixation method is designed easy to use,postoperative functional restoration of excellent,good appearance.We got satisfactory clinical applications.
