2244例流感样病例流行病学和临床表现分析

目的 分析到目前为止我院发热门诊筛查的甲型H1N1阳性与阴性流感患者的流行病学和临床表现有何差异.方法 2009-05-01～2009-11-30经我院发热门诊筛查的流感样病例2244例,其中甲型H1N1阳性患者337例,从流行病学和临床表现两方面进行回顾性比较分析.结果 2009年5～11月间甲型H1N1流感阳性患者在流感样病例中的比重逐渐上升,尤其到2009年11月其比例上升到将近当月流感样病例总数的1/3.流行病学方面,甲型H1N1阳性流感患者的接触史比例较甲型H1N1阴性流感患者略多.临床上,在伴随症状和体征方面两组基本相同,在病程方面两组比较差异无统计学意义;但在胸片改变、白细胞计数改变、重症和危重病例数及死亡例数方面,甲型H1N1阳性流感患者较甲型H1N1阴性流感患者比例明显增多.结论 我国已出现甲型H1N1流感病毒流行,甲型H1N1阳性流感病例在症状学和一般情况上与阴性病例很难鉴别,甲型H1N1阳性流感不论是从传染性还是毒力上看,其仍较甲型H1N1阴性流感为强,但其总体病死率仍较低.

Analysis of epidemiology and clinical characteristics of 2244 influenza - like patients

Objective To identify the differences in epidemiology and clinical manifestations between influenza virus A (H1N1) and H1N1 - negative influenza in china.Methods The Peking Union Medical College Hospital fever out - patient department has screened 2244 influenza - like fever cases from May 1, 2009 to November 30, 2009, in which 337 cases of H1N1 - positive patients were detected.Epidemiological and clinical features were analyzed retrospectively and compared.Results The proportion of H1N1 - positive patients in influenza - like cases gradually increased from May to November in 2009, especially in November it sharply increased to nearly 1/3.In epidemiological aspect, H1N1 - positive cases had a bigger proportion of exposure than H1N1 - negative cases.In clinical aspect, the symptoms and signs in the two groups was basically the same, there was also no significant difference in the course of disease in two groups.The chances of CXR changes, WBC count change, the number of severe/critical cases and deaths cases all increased obviously in H1N1 - positive cases.Conclusion The influenza virus A (H1N1) pandemic has emerged in china.H1N1 -positive cases are difficult to distinguish from the H1N1 -negative ones in symptomatology, but H1N1 -positive cases are more contagious and more toxicity, but its overall mortality is still low.
