难治性癫痫患者脑组织内致痫灶与其周围组织P糖蛋白表达量的比较

摘    要:目的:比较难治性癫痫患者手术切除脑组织致痫灶和灶旁组织P糖蛋白表达量的差异,探讨其在难治性癫痫发生过程中的作用,促进阐明难治性癫痫的耐药机制。方法:收集自2007年4月-2008年1月在首都医科大学宣武医院功能神经外科行"致痫灶切除术"的难治性癫痫患者,所有患者均在术前获得知情同意,术中使用神经电生理手段,区分致痫灶和灶旁组织;使用Western-blot蛋白分析技术定量检测P糖蛋白的表达量;用Image-J软件对蛋白条带进行分析,蛋白条带面积与灰度的乘积为分析指标;使用SPSS11·5进行配对t检验,分析其差异。结果:11例致痫灶内P糖蛋白与内参β-肌动蛋白(β-Actin)灰度和面积之积的比值(V-P/V-Actin)为0·69±s0·53,灶旁组织V-P/V-Actin为0·53±s0·39;P糖蛋白在致痫灶中表达量明显高于灶旁组织,P<0·05。结论:P糖蛋白在难治性癫痫患者脑组织致痫灶中的高表达,提示难治性癫痫耐药机制与P糖蛋白高表达相关。

Abstract:Objective:To identify the role of P-glycoprotein(P-GP) in the development of intractable epilepsy(IE) and promote the elucidation of the mechanism of IE by comparing the expression of P-GP between epiletogenic zone and tissue surrounding the zone in refractory epileptic patients. Methods:With neuroelectrophysiology technique,we distinguished the tissue where originate abnormal discharge from the tissue surrounding it in the refractory epileptic patients; all included patients were hospitalized in Xuanwu Hospital from April, 2007 to January, 2008, and obtained informed consent before the operation. Then, we quantified the expression of P-GP with Western-blot; after analyzing the protein band with software Image-J, we calculated the product of area and gray scale;with this product,we made a comparison through paired-samples T test with SPSS 11.5. Results:The product of area and gray scale(V-P/V-Actin) was 0.69±s 0.53 in epileptogenic zone, and V-P/V-Actin 0.53±s 0.39 in the brain tissue surrounding the epileptogenic zone. The expression of P-GP in the epileptogenic zone was significantly higher than in the tissue surrounding it, P<0.05. Conclusion:P-GP is highly expressed in the epileptogenic zone, which illustrates that P-GP is related to the drug resistance of IE patients.
