全数字化乳腺摄影与磁共振成像对乳腺癌诊断价值的比较

摘    要:目的比较全数字化乳腺摄影(full-fielddigitalmammography,FFDM)与磁共振成像(MRI)对乳腺癌的诊断价值。方法选取2003年4月-2006年4月期间行FFDM并怀疑有乳腺病变的100例患者作为研究对象,其中良性病变46例,恶性病变54例,均经病理证实。全部病例在一周之内同时行MRI检查。结果良性病变46例中,临床触及不到肿块6例,其病理结果:纤维腺瘤5例、腺病1例;临床触及肿块40例,其病理结果:纤维腺瘤25例、乳腺囊性增生症4例、乳腺腺病3例、乳腺结核1例、乳腺囊肿7例。恶性病变54例中,临床触及不到肿块8例,病理结果:癌前病变1例、导管内癌3例、浸润导管癌3例、黏液腺癌1例;临床触及包块46例,病理结果:浸润性导管癌44例,黏液腺癌2例。本组研究显示FFDM对乳腺癌诊断的敏感性、特异性和准确性分别为92.59%、91.30%、92%;MRI对乳腺癌诊断的敏感性、特异性和准确性分别为85.19%、86.96%、87%。结论全数字化乳腺摄影对乳腺癌的诊断价值优于MRI。

Abstract:Objective To compare the diagnostic value of full-field digital mammography (FFDM) and magnetic resonance imaging (MRI) in breast cancers. Methods 100 patients with breast lesions diagnosed by FFDM were collected between April 2003 and April 2006, including benign lesions 46 cases, malignant lesions 54 cases. All patients were proved histopathologically, which were examined with FFDM and MRI within one week. Results During benign lesions, there were 6 cases of nonpalpable lesions and 40 cases of palpable lesions. The results of pathology were respectively fibroadenoma 5 cases, adenosis 1 case, and fibroadenoma 25 cases, cyst hyperplasia 4 cases, adenosis 3 cases, tuberculosis 1 case, cyst 7 cases. During malignant lesions, there were 8 cases of nonpalpable lesions and 46 cases of palpable lesions. The results of pathology were respectively precancer lesions 1 case, intradutal carcinomas 3 cases, invasive ductal carcinomas 3 cases, mucoid carcinoma 1 case, and invasive ductal carcinomas 44 cases, mucoid carcinoma 2 cases. The specificity, sensitivity, accuracy of FFDM and MRI for breast cancers were respectively 92.59%, 91.30%, 92% and 85.19%, 86.96%, 87%. Conclusion FFDM is prior to MRI for breast cancers.
