脑内海绵状血管瘤的MRI和CT诊断

摘    要:目的分析脑内海绵状血管瘤的CT和MRI影像学特点。方法回顾性分析68例CT和MRI表现,并比较CT和MRI优缺点。结果海绵状血管瘤位于幕上55例,幕下脑干4例,小脑9例。68例病灶有出血54例,钙化17例。MRI检查。T1加权像病灶显示为短T1高信号,周边为轻度低信号影;T2加权像病灶中央呈高、低混杂信号影,周边为极低黑色信号环。增强有轻度强化或无强化。CT平扫为斑片状不规则形高密度区。增强后有轻度强化或无强化。结论应用MRI诊断脑内海绵状血管瘤,其敏感性高,特异性强。对脑内海绵状血管瘤的显示MRI明显优于CT。

Abstract:Objective To analyze the CT and the MRI features of cerebral cavernous angioma and evaluate diagnostic accuracy.Methods Sixty eight cases of cerebral cavernous angiomas(32 males and 36 females) were analyzed.Sixty eight cases examined by MRI,in which 48 cases were used contrast media.CT was performed in all cases,in which 32 cases were enhanced.CT and MRI findings were analyzed and compared retrospectively.Results Follow-up MRI were analyzed in order to detect morphologic changes,including changes in signal intensity,shape,dimension,and mass effect(16 of 20 patients);and to determine whether new lesions had occurred(20 of 20 patients).16 patients whose natural history was available had 27 lesions.Of these 27 lesions,19 had unchanged MRI features.Modifications were found in eight cases.Four lesions appeared to be decreased in size.Signalintensity changes were consistent with the usual degradation of blood products,such as the progressive disappearance of hypemintense areas on both short TR and long TRITE images,leading to a residual area of decreased signal intensity on bong TR/TE images.An increase in size was noted in the other four lesions,with concomitant signal-intensity changes.Conclusion MRI is more sensitive than CT for demonstration of cavernous angioma.
