PFNA与DHS治疗老年股骨粗隆间不稳定性骨折疗效对比

目的 分析股骨近端防旋髓内钉(PFNA)与动力髋螺钉(DHS)治疗老年股骨粗隆间不稳定性骨折的疗效。方法 选择2009年1月至2012年12月在山东大学齐鲁医院就诊的老年股骨粗隆间不稳定性骨折患者57例，随机分为PFNA组和DHS组，统计手术时间、手术切口、术中出血量、住院时间、负重时间、住院费用及Harris评分并进行比较。结果 PFNA组在手术时间、手术切口、手术出血量、住院时间、负重时间及Harris近期评分等方面优于DHS组(P＜0.05)。住院费用和Harris远期评分等方面两组无明显差异(P>0.05)。结论 PFNA具有创伤小、手术及住院时间短、下地负重早等优点，适用于伴有骨质疏松的老年股骨粗隆间骨折患者。

Curative effect analysis of DHS and PFNA in treating intertrochanteric femoral fracture

Objective To evaluate and compare the efficacies of proximal femoral nail anti-rotation (PFNA) and dynamic hip screw (DHS) in the treatment of senile patients with femoral intertrochanteric fractures. Methods From Jan. 2009 to Dec. 2012, a total of 57 senile patients with femoral intertrochanteric fractures were randomly divided into the PFNA group and DHS group. The operation time, surgical incision, blood loss, hospital stay, loading time, hospitalization costs and Harris scores of the two groups were compared. Results Patients of the PFNA group had better efficacy than those of the DHS group in operation time, surgical incision, blood loss, hospital stay, loading time and short-term Harris score(P< 0.05). There was no difference in hospitalization costs and long-term Harris score between the two groups (P> 0.05). Conclusion PFNA has the advantages of small incision, short operation time, brief hospital stay and quick recovery, which is the ideal therapeutic method for senile patients with femoral intertrochanteric fractures.
