经颅彩色多普勒超声在小儿颅内出血中的应用

摘    要:目的 评价经颅多普勒超声在小儿颅内出血中的诊断价值。方法 应用彩色多普勒超声诊断仪对41例颅内出血患儿及50例正常儿童进行检测。结果 41例颅内出血患儿中脑实质出血18例,单纯硬脑膜下腔出血5例,诊断符合率100%。蛛网膜下腔出血14例,诊断符合率78%。脑室出血4例,诊断符合率50%。多普勒显示小儿颅内出血血流频谱变化复杂,如高尖波、尖波、高振荡波等。脑实质出血以高尖波为主,蛛网膜下腔出血以尖波为主。结论 经颅彩色多普勒超声对小儿颅内出血有很高的诊断价值。

Abstract:Objective To evaluate the diagnostic value of transcranial color Doppler( TCDUS) ultrasound in the diagnosis of child cerebral hemorrhage in children.Methods Fourty one children of cerebral hemorrhage and 50 normal subjects were examined by TCDUS.Results There were 18 patients with cerebral substance bleeding and 15 with cerebral dura mater bleeding,the diagnostic rate of TCDUS was 100% .The diagnostic rate of 14 children with arachnoid mater bleeding was 78% .There were 4 patients with ventricle encephali bleeding, the diagnostic rate was 50% .The spectrums of this bleeding were varied including high sharp wave,sharp wave, and high oscillation wave. Cerebral substance bleeding mainly showed as high sharp wave while arachnoid mater bleeding showed as high wave. Conclusion TCDUS is valueble for the diagnosis of cerebral hemorrhage in children.
