低位直肠癌术后复发的原因分析

目的探讨低位直肠癌术后复发的原因及其预防措施。方法对我院1993年6月～2005年6月间手术治疗低位直肠癌66例的手术后随访结果,按年龄、病理分期、癌肿至齿状线的距离及手术方式进行比较分析。结果30岁以下年龄组病人术后复发早、生存期短;病理分期DukesC期病人较A期、B期病人的术后复发率高,癌肿至齿状线距离越近复发越早;年龄相同病期相同而手术方式不同,则无明显差异。结论病人自身条件是局部复发最重要的因素,不按照TME的原则和无瘤操作技术是手术后复发不可忽视的因素。

Causes of recurrence after resection of low rectal carcinoma and countermeasures

Objective To explore the causes of recurrence after resection of low rectal carcinoma and preventive measures.Methods According to different age groups,pathological stages,distances from cancer to dentate line and surgical procedures,the follow-up results of 66 cases of low rectal carcinoma receiving surgical treatments from Jun.1993 to Jun.2005 were compared and analyzed.Results The postoperative recurrence was early and survival time was short in the patients less than 30 years old.The recurrent rate in the patients with Dukes classification C was higher than that in those with A or B.The shorter the distance from cancer to dentate lines was,the earlier the recurrence was.The surgical procedures had no influence on the recurrence of low rectal carcinoma.Conclusion The general conditions of the patients are the most important reasons influencing the postoperative local recurrence.Not complying with TME rules and operative tumor-free technique is an non-neglectable factor resulting in the postoperative recurrence.
