2004-2013年河北省艾滋病ART病例死亡情况分析

目的分析艾滋病抗病毒治疗（ART）病例的死亡情况,探讨相关影响因素。方法对河北省2004-2013年死亡的艾滋病ART病例资料进行回顾性总结分析。结果截至2013年12月31日,共有229例治疗病例死亡,71.2%死于艾滋病相关疾病。男性病例占62.4%;40～49岁组病人占38.9%;经血途径感染者占40.6%,经性途径感染者占52.0%;农民占64.6%;初中及以下占81.2%;已婚或同居有配偶者占66.4%。40.2%的病例治疗前CD＋4T淋巴细胞（简称CD4细胞）计数〈50个/μL,且其中50%病例在3个月内死亡;而治疗前CD4细胞计数〉200个/μL的病例占死亡总数13.5%,仅22.6%在3个月内死亡。经血液途径感染者中,69.9%的病例生存时间超过3个月,经性途径感染的病例中,50.4%在3个月内死亡。结论治疗延迟是导致艾滋病病人死亡的主要原因,早期治疗能降低艾滋病病死率。

An analysis of death of patients receiving antiretroviral therapy in Hebei Province from 2004 to 2013

Objective To explore influence factors by analyzing the death causes of AIDS patients receiving antiretroviral treatment.Methods A retrospective analysis was performed in death patients receiving antiretroviral treatment in Hebei Province from 2004 to 2013.Results Up to December 31,2013,there were 229 death patients,with 72.1%died of AIDS related diseases.Male patients accounted for 62.4%,patients of 40-49 years old 38.9%,patients transmitted through blood 40.6%,patients through sexual transmission 52.0%,farmers 64.6%,patients at level of junior high school and below 81.2%,and married or cohabiting spouses 66.4%.The patients with CD4 cell counts less than 50cells/μL accounted for 40.2%at baseline,and 50%patients died in 3months.The patients with CD4 cell counts higher than 200cells/μL accounted for 13.5%,and only 22.6% died within 3months.Patients transmitted by blood with survival time more than 3months accounted of 69.9%,while 50.4% of sexually transmitted patients died within 3months.Conclusion Delayed treatment was the primary cause of death,and an earlier treatment would decrease the death rate.
