摘    要:目的 了解呼吸科重症监护病房(respiratory intensive care unit,RICU)患者医院获得性感染的细菌分布及耐药情况,为临床合理使用抗生素,预防医院感染提供依据.方法 调查我院2005年1~12月RICU发生医院感染患者的临床资料.结果 RICU医院感染率为64.10%,以下呼吸道感染为主占65.19%,病原菌以革兰阴性杆菌为主占74.34%.其中铜绿假单胞菌、鲍曼不动杆菌分别占28.68%、23.02%,革兰阴性杆菌对亚胺培南的耐药率最低为35.94%,而对其它抗菌药物的耐药率均较高.结论 应重视病原菌的培养和药物敏感实验结果,合理使用抗生素.

Abstract:Objective To determine bacterial spectrum and drug resistance of respiratory intensive care unit(RICU) patients with hospital acquired infection and to provide the basis for proper use of antibiotics and to prevent RICU patients from hospital acquired infection.Methods Bacterial spectrum and drug resistance of all the RICU patients with hospital acquired infection between Jan and Dce 2005 were analyzed retrospectively.Results The nosocomial infection rate was 64.10%.The lower respiratory tract was the main infection site,accounted for 65.19% of the patients in RICU.The pathogen of hospital acquired infection was mainly Gram-negative bacilli,accounting for 74.34%.The leading pathogens were Pseudomonas aeruginosa,accounting for 28.68% and acinetobacter baumannii(23.02%).The resistance rate of Gram-negative bacilli for emipenem was the lowest(35.94%).but the other antibiotics resistance rate were higher.Conclusion Antibiotics should be used rationally according to the pathogen cultures and drug sensitive test.
