甲状腺肿瘤侵犯气管39例外科治疗分析

目的对甲状腺肿瘤侵犯气管及其外科处理原则进行探讨。方法回顾性分析我院8年来对39例甲状腺肿瘤侵及气管的外科治疗情况。结果甲状腺良性肿瘤20例，恶性肿瘤19例，术前均有呼吸不畅或影像学发现有气管受压、移位或狭窄，术中证实气管受侵22例，术后早期出现呼吸道梗阻7例，死亡4例。结论长期肿大的甲状腺肿以及漫润性的甲状腺癌均可造成气管受压、软化或狭窄，术中术后均可发生急性呼吸道梗阻，甚至窒息死亡。

Analysis of Surgical Treatment of 39 Cases with Thyroid Gland Tumor Invading Bronchial Tract

Objective To searchthyroid tumor invading windpipe and the principles of surgical treatment. Methods We looked back the conditions of surgical treatment about thirty-nine cases of thyroid tumor encroaching on windpipe that our hospital experienced in the past eight years. Results There were twenty eases of thyroid benign tumor, nineteen malignant tumor. The phenomenon as not breathing smoothly occurred before operations. The windpipes were pressed or narrowed by imaging. It was confirmed that there were twenty-two cases whose windpipes were invaded. There were seven eases of respiration obstruction during the earlier post-operations. There were four deaths. Conclusion The thyroid which is always swelling and is infiltrated can all make windpipes pressed, softened or narrowed, respiration obstructed or even suffocation death all occur during operations or afteroperations.
