利伐沙班与低分子肝素对髋关节置换术后深静脉血栓的预防效果的 Meta 分析简

目的：比较利伐沙班与低分子肝素预防下肢深度静脉血栓（DVT）的疗效，对其安全性进行评价。方法：计算机检索Embase、CBM、VIP、CNKI、WanFangData、Pubmed和中国期刊网等数据库，收集利伐沙班与低分子肝素对髋关节置换术后DVT预防效果的随机对照试验，检索时限从建库至2013年7月。结果：共纳入随机对照试验文献6篇，患者756例。Meta分析结果显示，利伐沙班组与低分子肝素组术后DVT发生率分别为6．04％和9．33％，无统计学差异RR=0．63，95％CI（0．36，1．08），P=0．09]；利伐沙班组与低分子肝素组术后出血发生率分别为2．55％和4．64％，无统计学差异RR=0．56，95％CI（0．17，1．84），P=0．34]；两组药物术后伤口引流量无统计学差异（P〉0．05）。结论：利伐沙班对髋关节置换术后深静脉血栓的预防效果与低分子肝素相当，无统计学差异。

The Meta - analysis of rivaroxaban versus low molecular weight heparin to prevent peep venous thrombosis after hip arthroplasty

Objective：To compare the effects of rivaroxaban and low molecular weight heparin on preventing deep venous thrombosis after hip arthroplasty, evaluating their safety. Methods ： The databases such as Embase, CBM, VIP, CNKI, WanFang Data, Pubmed and China QKing were searched from the date of their establishment to July 31st,2013 to collect the randomized controlled trials about the effects of rivaroxaban and low molecular weight heparin on preventing deep venous thrombosis after hip arthroplasty. Results ： A total of 6 randomized controlled trials involving 756 patients were included. The results of meta - analysis suggested that the incidence of symptomatic DVT between the two groups was similar RR = 0.63,95% CI （ 0.36,1.08 ） ,P = 0.09 ] , so did the incidence of bleeding RR = 0.56,95% CI （ 0.17,1. 84 ） ,P = 0.34 ] and the drainage volume （ P 〉 0.05 ）. Conclusion ： Comparing with low molecular weight heparin,rivaroxaban had the same effect to prevent DVT after hip arthroplasty, there was no statistically significant difference between the two groups.
